• Valid, unexpired driver’s license
• Proof of current address if different from address on identification
• Verification of all income:
  • 2 most recent paystubs
  • If self-employed- 2 years of taxes (Schedule C)
  • Military- LES
  • Social Security Award Letter (If Applicable)
  • Statement of Assets – Bank statements
• Current Home Owners Insurance Declaration Page
• Current Home Mortgage Statement
